I have checked your issue.
But according to this information I am not able to find the problem.

Would you please provide more information on this like "- openerp-server
log" in DEBUG level, when creation fails or proper steps.

You can try same as with the version 6 of OpenERP and notify us you
still faced the problem or not in v6.

Title:
  create base possible only if server is restarted

Status in OpenERP Modules (addons):
  Incomplete

Bug description:
  _Environnement_: windowsXP sp3+VMWare Player 3.1.4 build-385536 +Ubuntu WS 10.10+Openerp 5.0.14 (Ubuntu embedded). Installation successfull. Postgres and openerp-server running and communicating. Postgres user/role openerp created (superuser, creatdatabase. norolecreate).  Ubuntu openerp client (embedded). 
  _gravity_ annoying, nonblocking. Reproducible.
  _Symptoms_ First connections says "No database found, you must create one!". When creating base get the message "cannot create database" (approximatively).
  _Cause_ I don't have a clue.
  _Workaround_ Restart openerp-server with : "/etc/init.d/openerp-server restart" and then recreate base. It then creates base happily. Check: remove base, stop and start system, same problem again, same workaround.
  _Comment_ I keep this virtual machine by me (4.9 GB). If you need it I send it you via download free.fr
  I saw a comment on openerp forum from somebody who's got same problem with earlier version: 
  http://www.openerp.com/forum/topic20447.html  He solved it with the very same solution as mine.
