[Bug 2016850] [NEW] results reporting does not indicate test case version

Public bug reported:

It occurred to me today that when somebody reports test results we don't
know which version / revision of the test case they were following.
Subsequently, if the test case were to change (without a new build
version / respin) then we could have results reported about two
different versions of the test case and not know which version the
tester was using.

** Affects: ubuntu-qa-website
     Importance: Undecided
         Status: New
