  Rename the template file

Status in MLHIM Constraint Definition Designer:
  Fix Committed

Bug description:
  In order to avoid confusion regarding CCD and CDD.  Rename the
  template file to CDD-<version #>  then always refer to the graphical
  version of a constraint definition as the CDD.  A CCD is created from
  the conversion of teh XMind file to an XML Schema.
