I updated my 12.04 64-bit installation a few days ago and I'm still
seeing disappearing firefox window decorations.  I'm using gnome
fallback with compiz.  It only happens when I click on a link from
within another application like thunderbird or tomboy.  It does not seem
to matter if firefox is maximized or not.  Workaround, as described
above, is to minimize and then unminimize firefox from the bottom gnome
panel.  Nothing in /var/crash.
