I have the same problem using 6.1 rc1. When I create a new database
without demo data than I get this error. I have tested this with dutch
chart.

Bug description:
  HEllo.

  Installing Account apps trought web give this error:

  "Tax Name must be unique per company! "

  IMHO, we are having an error on excecute method on install wizard.
