Title:
When using NVIDIA driver wrong resolutions are picked causing *out of
range* error from monitors OSD and cannot display analog signal from
VGA output to TV because of refresh rates

Status in nvidia-graphics-drivers package in Ubuntu:
  Invalid

Bug description:
  Binary package hint: xorg

When using NVIDIA driver, wrong resolutions are picked causing *out of
range* error from monitors OSD and cannot display analog signal from
VGA output to TV because of what appears to be incorrect refresh rates

  The fix is to include

Option "ModeValidation" "NoXServerModes" in xorg device section for
  card0

This needs to be standard in the distribution with installation of
  nvidia drivers.
