I usually got Firefox open with at least two windows. This gets stored
by Firefox as well as the contained tabs so when I restart Firefox, all
windows get restored.

Recently (and this has NOT always been the case!) in this situation the
second window has its menu bar INSIDE the Firefox window, not in the
Unity single menu bar on top. This looks odd and strange and is really
annoying as this only affects one of the two windows. (see attached
picture)

The situation is reproducable. When I open a new window in a running
instance, everything is fine - the window's menu is not there as it
should be.

But when I stop  Firefox and restart it so it restores all former
windows, the problem is back again!
