[Bug 1525392] [NEW] unable to use Sprint/Franklin U301 modem (16d8:6008, CMOTECH CMO6008) on modern Ubuntu (Wily/Xenial)

This modem is quite old, its firmware date is from 2008 (as shown in modem-manager-gui)
In NetworkMnaager, it adds 'mobile broadband' icon to the menu, with enable/disable checkbox (which is changed on clicking it).
In NM Connection Editor it allows me to create Mobile Broadband connection, it sees CMOTECH CDMA modem device, and allows me to select country and operator (Ukraine/InterTelecom), which fills dial number, login and password with good-looking strings, but then I was never able to connect to this connection (connect/disconnect buttons are disabled/greyed-out).

I notice that ModemManager gets unexpected QMI service versions for dms and nas svcs.
I plugged it into Win10 system, and it worked after minimal manual setup (phone number #777, login and password given by operator)
I tried to replace modemmanager with wader-core but I could not make it work. Returned back to modemmanager.
Please advise what to do next.

I notice that ModemManager gets unexpected QMI service versions for dms and nas svcs.
I plugged it into Win10 system, and it worked after minimal manual setup (phone number #777, login and password given by operator)
I tried to replace modemmanager with wader-core but I could not make it work. Returned back to modemmanager.
Please advise what to do next.
