This should be fixed in Totem 3.1.0.  From it's news file on the GNOME
FTP server:

* Add ability to "thumbnail" DVD ISOs

Bug description:
  Binary package hint: totem

  It would be nice if the thumbnailer could examine ISO files for DVD or
  VCD images, and extract a thumbnail from the internal video (perhaps
  defaulting to title 1, or the longest title).
