It seems that if firefox starts maximised and then is reduced to a
normal window the bar is missing.  If it starts as a window the bar is
there.   Firefox seems to want to always start maximised even though I
almost never use it that way.

  Following the latest round of updates (as of 28/10/2011) when either
  thunderbird or firefox start they have no window decoration.
