I can also reproduce this, by opening a document, fullscreening
LibreOffice, closing it, and opening another (or the same) document
(again). This way no title is set in the upper panel of Unity.

  When ever I opened 2 or more libreoffice document, the title bar (that
  means the integrated menu bar didn't recognise the libreoffice
  document opened) for the newly opened document window is missing. This
  happened to the unity, not unity-2d.
