Title:
Window buttons sometimes don't appear in LibreOffice

Status in “libreoffice” package in Ubuntu:
  Confirmed

Bug description:
This is quite a frustrating bug in LibreOffice that has only occurred
for me since Ubuntu 11.10. When opening any LibreOffice app (Writer,
Impress, etc.) there are sometimes no window buttons (Close, Minimize,
Maximize) in the top-left corner. The only way to close the app is to
navigate to File > Exit inside LibreOffice.

This also has a far more frustrating side-effect: the app does not
appear in the Alt+Tab application switcher OR in the Unity Launcher.
This means I have to switch to the app using Ctrl+W then clicking on
  it.

I cannot reproduce this bug right now but will attach a screenshot
  when it happens again.
