Can you see if changing the icon theme back to ubuntu-mono-* helps? If
yes, then its a gnome-icon-theme issue.

Bug description:
  I'm using the "gnome" icon theme, which may not include the icon for
  banshee. When upgraded to 11.04, I get a black square for banshee in
  indicator-sound, and I don't know how I can fix this.
