Fred LeMaster, thank you for reporting this bug to Ubuntu. Hardy Desktop reached EOL on May 12, 2011.

Bug description:
Binary package hint: evince
If you create a custom paper type where the page width is larger than
the page height evince will always orient the paper so that the longer
dimension is the height. Switching the orientaion to lansdcape will
make the dimensons correct but then the pdf file is sideways.
To replicate:
Try creating a custom paper type that is 5 in x 3 in. Print a document that is wider than it is tall. The resulting pdf file will have your content jammed at the top of a page that is 3 in x 5 in.
