Child Health Record - ideas in Viet Nam

Dear all,

We're going to build Child Health Record based on DHIS. I send the document
to represent our ideas about the module.

Please enclose the attached file and give us suggestion about the system.

According to our ideas, we try to create a form from system, but we can't
share data. Now we're researching how  to solve this big program.
