You may generate orgunits at any level on the fly by setting the map source
to "Database" as long as you have orgunit coordinates in your database.
There is now GUI for coordinate input in DHIS.

It is definitely possible and transparency is already an option when you
register overlays. Please go ahead and play with the overlays to see the
result. As you say, they must be managed properly and the background is a
problem.
