I think we should remove the uniqueness constraint completely, for both
orgunit name and shortname (and perhaps replace it by functionality to
easily list duplicate names, so that users can decide if it is ok or
not). Identification should be independent of name (to aid with
identification, one will often include things like geographical
coordinates and orgunit type at the facility level).

Bug description:
Currently, the name field in organisationunit have been set to be unique. This uniqueness constraint should only apply within the scope of the parent, i.e. it is the *combination* of the "name" and the "parentid" fields that should be unique.
