Status in DHIS 2 - District Health Information Software: Fix Committed

Bug description:
After upgrading to 2.0.3, one of the data entry forms refuses to show. It's just this one form (in DHIS for Gambia: Health Facility Quarterly Returns). The rest of the forms work as normal.
