Bug description:
  When designing a data entry form on the design data entry form page,
  clicking on the Save button saves the form the first time.
  Subsequently, during that same session, clicking on either the Save or
  Save and Close button generates the error message: A dataset with this
  name is already registered. Once this message is displayed, clicking
  on the Save and Close button does not close the data entry form.
