I was able to reproduce on t <http://dhis.uio.no/dev/>he devs branch, rev
3018, on the Development db. These were the steps:

1. Created a new category combination "test" with the following categories:
  - 15-49yrsPreg+15-49yrsNonPreg
  - ART Patient previous +current month + shif during month
  - At PHUc& In Community

2. Created three data elements with the above category combination
  - ABC_test1, ABC_test2, ABC_test3

3. When trying to create a dataelement dimension report table, only
ABC_test1 appears in the list when the category combination "test" is
selected.
