  minmaxdataelement table doesn't exist

Bug description:
  Hi

  I don't know what happening here. I can't open data entry form, it
  doesn't show anything and no message. In the tomcat it said that the
  minmaxdataelement table doesn't exist. I attached tomcat log file. If
  anyone know how to solve this problem, please help me
