The tablename field in reporttable table has been removed. This
exception is thrown because that field somehow has not been removed by
the startup routines. This is admittedly a problem but can't reproduce.
Please remove the field manually.

Bug description:
  I am trying to add a report table for the first time to my DHIS
  instance and it keeps generating an exception
