When I create data entry form, I insert data element into it, it show short
name in the text box. But after I save it, and edit it again. It doesn't
show any more. there is not any way to see the name or short name of that
text box unless search for the data element id in html source.

  After save Program stage custom data entry form, I open it again to
  edit, all the short names are invisible, data element fields are
  blank.
