  patient module - can't save design data entry form in first time

Status in DHIS 2 - District Health Information Software:
  Fix Committed

Bug description:
  When the data entry form is design for the program stage first time, it can't be saved. 
  Only after click design data entry form button again and design the second time, it can be saved.
