Title:
  adding and then removing a reminder from a program stage leaves the
  send-to field behind

Status in DHIS 2 - District Health Information Software:
  Fix Committed

Bug description:
  Editing a program stage leaves the send_to field and the combo-box
  widget behind. doing this multiple times leaves behind multiple
  widgets. I would suspect the bug is located somewhere in either the
  velocity template updateProgramStageForm.vm or the .js file included
  in that.
