Hi,

Just tested on 2.12, seems to work fine.

http://apps.dhis2.org/demo/dhis-web-dataentry/index.action

Can you access that url, login with admin/district, and select
DS_TEST_OPTION_SET and see if it renders ok?

What browser are you using, and version?
