Hi Jason

Hi I have attached the .csv I am uploading.

I have written a small program that maps organisation name ,
dataelements and categoryoptioncombo to uids. The import module says
that the import has been sucessful but when I open the data entry forms
nothing appears.

Can you give a bit more clarity (perhaps with an example) of what you Excel
file looks like?

In general, DHIS2 cannot import Excel directly but rather CSV files. The
format is described in detail here.
