  deletion of category option default

Status in DHIS 2:
  Fix Committed

Bug description:
  DHIS currently allows for deletion of the "default" category option.
  This should not be possible, in the same manner as it is not possible
  currently to delete the default category or category combination.

  This then causes mess in data element having default category combo.
