  "cancel" button not functional in add new entity profile

Bug description:
  Trying to add a new entity instance for enrolment into a program,
  cancel button doesn't work one has to find other means of etting to
  the previous screen. To test,

  1) go to Find/add

  2) Click add new

  3) click cancel

  4) Nothing happens
