[Bug 1300614] [NEW] cannot have two date fields in a custom data set form
Public bug reported:
Steps to reproduce on the demo server.
1) Create two data elements (Test date 1 and Test date 2) both with type of "Date"
2) Add these two a new dataset called "Date test" and assign to SIerra Leone. Be sure it is assigned to the user as well.
3) Design a custom form with a single table with two rows and 1 column.
4) Insert both data elements into the form, one in each cell.
5) Go to data entry, and select Sierra Leone. Enter data for any period for both dates. It works.
6) Choose a new date. You can only enter data into the last field which you selected the first time you entered.
