  program stage custom form not pullingthrough with build upgrade on
  version 2.15

Status in DHIS 2:

Bug description:
  I have been using Version 2.15 from Trunk. On one of the latest build
  upgrades, all my program stage data entry forms did not pull through.
  The dataentryform table still has entries for the Program stages but
  there's nothing in the front end.

  This problem started at the same time this error started showing up
  with Tomcat startup:
