Agree with Olav in this case.. I think the double arrow selection should
follow the drop down selection above, if set to all then all should be
selected, if set to a group, then all in group should be selected. The
reason for current behavior is that we have progressive loading of items -
sometimes we have thousands of category options e.g. in the pepfar system.
So selecting all would require some special handling for going to the
server and fetch all items (which might sometimes actually be tens of
thousands and potentially freeze the browser).
