It works now but prob to acces my form. the error is:' No forms found assigned to current user' while the form is assigned to my user role!!

afraid that web based smartphone client currently
doesn't support option lists.
You can
use this app in order to fill reports: https://play.google.com/store/apps/details?id=org.dhis2.mobile
Do not hesitate to ask anything regarding
it.
