Hi Seth

This kind of errors produce a log on your server, could you please have a
look and if so, please give us the output?

Also, please know that you are using a module that has already been removed
in later versions of DHIS2, and replaced by the tracker capture app (DHIS 2
is currently on 2.22), so this module won't be getting any updates in the
future
