Title:
  Event Capture not switchig to offline mode

Status in DHIS:
  Incomplete

Bug description:
  1)  event capture failing to load structure (CSS i think) when the server goes offline
  2) the application fails to bring login screen when the server is back on line
  3) the application no longer notifies the user on status online/offline
  4) the application no longer prompts the user to upoload data saved while offline.
  5) manifest error: Application Cache Error event: Manifest fetch failed (4) http://localhost:8082/dhis-web-event-capture/cacheManifest.action
