Hi Pedro,

Could you try to load the form with the browser console
<http://webmasters.stackexchange.com/questions/8525/how-to-open-the-javascript-console-in-different-browsers>
open.
Are there any exceptions?

Also, are you running DHIS 2 behind a caching proxy? If so, nuke the proxy
cache (as well as the browser cache). Sometimes static assets (scripts and
so forth) stick around in cache between upgrades.
