Hi Krzysztof,
This normally results from the category options being modified after the
category combo has been created, especially if this has been done directly
in the database.

There is a maintenance function in the Data Administration app

which will generate category option combos (and remove one which no longer
should exist).

Did you happen to manipulate the category or options directly in the
database after it had been created?
