As Lars wrote we are working on simplifying the process to customise
registration/enrolment forms.
In the current trunk (2.13 snapshot) you can already create custom
registration forms where you decide what to include, but we will try to
simplify the way this is done before the 2.13 release.

If you want to test this right now you can give it a try on our demo server
for the trunk version:
http://apps.dhis2.org/dev (it is rebooted on every change so not always up)

In Maintenance->Persons and Programs there is a menu item called Person
Registration form.
Look for your program and click on the Design Data Entry form operation on
the right hand side (second icon).
In the designer you can use exactly the attributes and identifiers you need.
You can also specify that you want to assign a default value to an
attribute and then hide it from the form.
E.g. you may want to set gender=female for an ANC program, but not want the
user to have to type this in on every registration.

For the 2.13 release we will make the hide and default value options
available also to "default" registration forms, so that you don't have to
work with the html designer to achieve this.
