Hi ,

I am trying to configure patient tracker on a test instance, The test instance is online and working ok. What I am struggling with is getting the registration form to work. I have setup my initial option sets, my data elements as well as program and program stages however when I attempt to register the form is unavailable I get the below captured screenshot. Anyone kindly help.
