Thanks for your response, am sending you a couple of screenshots to show what I have setup (attributes/stages/data elements/program/)

Despite having done this, the form is still not available.

Hi Jacob,

Have you created attributes and assigned them to your program?

Also, for a program to be available for registration/enrollment, it needs to have attribute(s), stage(s), and stage(s) with dataelements.

If all this is in place, see any log pressing F12 (if you are on chrome).

I am trying to configure patient tracker on a test instance, The test instance is online and working ok. What I am struggling with is getting the registration form to work. I have setup my initial option sets, my data elements as well as program and program stages however when I attempt to register the form is unavailable I get the below captured screenshot. Anyone kindly help.
