Line-listing form

Hello everyone,

I would like to implement àaline-listing form to register Births and Deaths in Facility. (Name of child, Village, Sex, Age, Date of birth/Death, weight, breast feeding)

Please can u tell me step by step how to do it with DataSet And Data Element ?
