I am trying to enter patients information when entering the primary health care center like ( name , last name , age , height …etc.)

 

I did my form and specified date ( frequently is daily in data set) but only can enter one entity 

 

If another entity need to be enter it over write the old one 

 

Any advice would great

This gets easy when you use tracker/event based reporting, but daily depends on the entity you are entering data, if it is entering data for different entities in one day/categories can help, this is possible but one entity more than one time per day??? I am not sure. This is suitable for event based.
