Hi Flora,

In *Design custom entry form* of program stage, there is a button called *Other
program stages* in *Data element* form. You can click on this button to add
data elements from other stages.

In other stage, click on the *Load existing form* button and select the
custom form you created before.

Please see the snapshots.

I hope this is the thing you need.
