currently the way it works is that a data element can only have one category combination. This means that all "fields" in the form for that data elements will have the same data type.
The best way to achieve the form layout above is to have 2 data elements:
- One with numeric value type, and a category with three age groups.- One with date type and default category.
Then put these fields into a custom data entry form.
