I just had a quick look at the tracker app on one of the Rwanda dhis2
instances (it is running 2.17 built on  2015-01-21 02:05 - strange I don't
see the build revision)

The form has some attributes and the Enrollment date and Reporting date at
the bottom.  When I try to save I see that the following is POSTed.

Note there are no dates in this even though they were entered on the form.
This seems wrong,  I guess this is what causes the sql to fail.
