Analytics table process failed

Hi,

Has anyone come across the challenge described below?

We are having issues with data entry and approval(For an org unit - og
Ilaro State Specialist Hospital, for a specific period - November,2014 and
a specific dataset- TB/HIV). It is pertinent to note that, this is not the
case for other org units in the same period and for the same dataset.

We are running DHIS version 2.18, build revision 18402 and using Google
Chrome Version 42.0.2311.135 m. We are using PostgreSQL database.

Attached is a readable screenshot of the error message that comes up when
complete button is clicked on the data entry page.

Also, attached  are readable screenshots of the error message that comes up
when approve or unapprove button is clicked on the data approval page.

The following has been done but hasn't solved the issue:

   - Analytic Table Update
   - Resource Table generation
   - Server restart
   - Period pruning


Also, we keep getting a message saying monitoring process failed.

Please advise.
