"Survey" and "Outbreak summary" at country level

 

Dear all, 
 
We are in the process of translating into DHIS2 some questionnaires which would be shared with countries in order to understand better the status of our diseases in each country:
-        Is this disease notifiable?
-        Is there a surveillance system in place?
-        Is drug A available for treating patients?
-        …
 
We have also developed another form related to “Outbreak summary”. At the end of the outbreak, the country will be supposed to share with us
-        Number of cases, by gender and age group
-        Number of death, by gender and age group
-        Likely transmission route
-        Measures taken
-        …
 
We were initially thinking in developing these forms as “programs” and “events” as these information will be collected punctually (as a snapshot), it does not really represent aggregated data as a sum of individual data, but rather collective data, we were happy with the possibility of having program rules (skip logic)…
 
But at the time of analyzing these data, we face several problem, and notably two major issues
-        GIS App. It is not that straightforward to represent in a map the yes/no questions at country level
-        Disaggregation by gender and age group is largely more complicated than in datasets
 
I am therefore wondering if some of you have already experienced this kind of questionnaires, and what was your final decision (dataset versus program). In case you have documentation, we would be very happy to read it.
