I have tried to import the events after adding the new columns (completedDate and completedBy) and a date for completedDate. 
The process failed.
It seems it keeps in memory something from the last attempts, but we can't see anything in the DB. It also mentions that a trackedEntityDataValue and a programStageInstance are already created in the session (see screenshot).

When going to the Event capture App, instead of writting "There is no event" as it usually does when there is no event, 
it is blank (see screenshot). for sure it keeps trace of something somewhere...

Yes, please put them at two last columns as in order "completedDate", "completedBy" where completedBy is user name of person who completed the event.
