I suggest a modification of the copyright notices at the top of each
file. I think the example included below is both compact and
informative. State your opinions.

In a separate post, I will send a list of corrections/updates to the
actual copyrights.

// Copyright (C) 2003-2005 Johan Hoffman, Johan Jansson, Anders Logg.
// Licensed under the GNU GPL Version 2.
//
// Modified by Erik Svensson, 2005.
//
// First added:  2003-06-05
// Last changed: 2005-06-05

Note 1:

The author(s) who originally wrote the file has copyright (if
the original file contains something substantial, more than just a
template).

Note 2:

Each person who modifies the file adds a note "Modified by ..., year."

Note 3:

Copyright holder(s) don't need to add this note. Just update the years
of copyright.

Note 4:

Each file knows when it was first added and last changed.
