Hi

I have updated the electromagnetic TM cutoff mode demo somewhat by moving it
to the demo folder and renaming some of the variables.  I have also made
sure that the lines don't exceed 80 characters and included an initial
document to describe the problem being solved (was easier than doing it at
the top of the code).

Things I still have to do:
* Make use of petsc to solve the eigensystem
* Implement a C++ version of the demo
* Finalize the document
